Shadow's Privacy Policy--Must Be Read By All Prospective Volunteers This Notice sets forth the privacy policies and practices of Shadow Home Association.  This notice identifies the sources of nonpublic, personal information Shadow Home Association. collects regarding the potential, present, and former residents of the home provided by Shadow Home Association, and specifies what measures Shadow Home Association will take to secure the information.  The words "you" and "your" mean the past, present, and potential residents of the home provided by Shadow Home Association and the families and dependents of such residents. "Nonpublic information" means information about you that is collected in connection with your application for residency in Shadow Home Association and your actual residency in the home provided by Shadow Home Association.  Such information does not include information that is available from public sources, such as telephone directories or government records. In order to determine if you qualify to reside in the home provided by Shadow Home Association, or to assist you while you reside in home provided by Shadow Home Association, the staff of Shadow Home Association will collect information related to your mental and physical condition, medical records, criminal record, if any, public assistance and other assistance you receive or may be entitled to receive, information about your conduct while residing in the residence, your current employment, your job history, education and school attendance records, and all information deemed necessary or reasonable by the staff of Shadow Home Association to determine if Shadow Home Association can assist you and also insuring Shadow Home Association meets your needs while residing in the home provided by Shadow Home Association. The staff of Shadow Home Association will limit access to all nonpublic, personal information which is collected about you only to the staff, officers, directors, and attorneys of Shadow Home Association who need to know the information in order to make decisions concerning whether you are a candidate for residency, decisions related to your actual residency, and the ability of Shadow Home Association to assist you before or during your residency in the home provided by Shadow Home Association.  Shadow Home Association will maintain physical, electronic, and procedural safeguards that comply with federal regulations to protect your nonpublic, personal information from unauthorized use.  You may be requested from time to time to waive the release of such information to allow Shadow Home Association to assist you before or during your residency in the home provided by Shadow Home Association.  Shadow Home Association. will not disclose your nonpublic, personal information to nonaffiliated third parties without your consent, except as permitted or required by law.
